Sunday Training EVERY Sunday: Place: Honor Roll School, 4111 Sweetwater Blvd. Sugar Land, TX 77479 Time 1: 3:30 PM - 5:00 PM (ages 6 – 11) Time 2: 5:00 PM - 6:30 PM (ages 12 – 16) Option 1: $110.00 + $10.00 One-Time Registration Fee - Four (4) 90 Minute Sessions -Some months will include 5 Sessions; no extra charge for months that include 5 Sessions. -All sessions must be completed within the time allotted at the time of registration. Absences will be considered on a case by case basis. -Customers who sign up after the first session of the month will be quoted a price at the time of registration based on the number of sessions remaining in the month. Option 2: $35.00 Weekly + $10.00 One-Time Registration Fee - One (1) 90 Minute Session -Slot not guaranteed if paying weekly.

I assume the risk and hazards associated with his or her participation in all basketball training activities. I hereby authorize the staff of Larry Micheaux Basketball Academy to act for me in the case of any emergency requiring medical attention. I hereby waive and release Larry Micheaux Basketball Academy and its staff, employees and volunteers of any and all liability. I consent to all video taping and photographing during training. I agree that no compensation will be paid to me or my child for the use of these images.
